Everyone,

Below you will find a list of all of those attending Camp Hale.

To the far right of each name the status of your Medical Form is indicated.

Here is a quick guide to the meaning:
"Y" means that we have on file the proper Medical Form.
"C1" means that you only have to update the form we have on file.  (I will have that with me tonight at the meeting so please be sure to see me to take care of that).
"C2" you need a Class 2 Medical Exam - Medical Practitioner Signature Required.
"C3" you need a Class 3 Medical Exam - Medical Practitioner Signature Required.

If you have turned in your form lately, but it is not showing up properly on the list, it is possible that it has not been given to me.  Please send me an email letting me know.

If you have been in the Troop for a while and you would like a copy of your old records to help in filling out a new form, send me an email and I will get that to you.

If there is a Medical Practitioner (Doctor, Nurse, Nurse Practitioner, etc.) in the Troop that would be willing to do the basic exam, please send me an email so we could coordinate the time to do that at a troop meeting.  If you want more info on what this entails, feel free to email or call me.
